Rarely seem totally well

I have lupus (SLE) and I am on Rinvoq (immunosuppressant) and Hydroxycholorquine for over a year now. I tried Methotrexate (4 days a week in bed) and Imuran (fatigue, mouth ulcers and kidney problems). I am now on 2.5mg prednisolone which I am trying to get off of but I just feel very tired without it.

Despite doing all the right things I catch cold or get some weird infection regularly and barely have 5 days feeling really on top of the world before the next thing hits.

I hurt my back 5 weeks ago - nothing is broken but it is too sore to walk. I know I have to get fit but I can only walk about 5 mins at a time before I have to sit down again. I am trying to eat an anti-inflamatory diet but break out sometimes. I am incredibly tired even after a good night's sleep.

Any advice on how to get out of this downward spiral?
